Your Custom tailored Estate Plan will provide you with all the documents you need to ensure your loved ones are protected. (Loved ones meaning family as well as assets). These are the documents you may select:

  • Will and Testament (may be used as a back up to your living trust
  • Healthcare Power of Attorney
  • Power of Attorney for Financial Matters
  • Real Property Transfer Deed
To set up your trust, you will need to:
  • Select a successor trustee, the person who will carry out your wishes in the trust upon your death. You may also select an alternate successor trustee. You will need to provide their full names and current addresses.
  • Select your beneficiaries and decide what percentage of your estate each one will receive. If there are any specific items you want to designate, you will need to list them for us. You may also name alternate beneficiaries.
  • If minor children are beneficiaries, you may want to set up a sub trust to manage their property until a certain age. ie. ½ at age 21, balance at age 25; or 1/3 at age 21, ½ at age 25 and the balance at age 30.
  • Provide a list of your assets.
  • Fund the trust, that is, transfer the title of your assets to the trust so you no longer own them, the trust does.
To set up your will, you will need to:
  • Select an executor (should be the same as the Successor Trustee if you have a trust)
  • Select your beneficiaries. Provide their names, addresses, dates of birth and social security numbers
  • Provide the names of all living children from all relationships along with addresses, dates of birth and social security numbers
  • Select a guardian for your children in case you and your spouse pre-decease them.
  • Decide if you want to include any special instructions.
For your powers of attorney, you will need to:
  • Select the person you wish to act as a conservator for financial matters if you become incapacitated while living. You may also choose an alternate. You will need to provide us full names and current address.
  • Select the person who will act as your health care agent if you become incapacitated while living. You may also choose an alternate. You will need to provide us full names and current address.
